It is only safe to run ast_register_cleanup callbacks when all modules
have been unloaded. Previously these callbacks were run during graceful
shutdown, making it possible to crash during shutdown.

The menuselect conflict between app_voicemail and res_mwi_external
makes it hard to package 1 version of Asterisk. There no actual
build dependencies between the 2 so moving this check to runtime
seems like a better solution.
The ast_vm_register and ast_vm_greeter_register functions in app.c
were modified to return AST_MODULE_LOAD_DECLINE instead of -1 if there
is already a voicemail module registered. The modules' load_module
functions were then modified to return DECLINE instead of -1 to the
loader. Since -1 is interpreted by the loader as AST_MODULE_LOAD_FAILURE,
the modules were incorrectly causing Asterisk to stop so this needed
to be cleaned up anyway.
Now you can build both and use modules.conf to decide which voicemail
implementation to load.
The default menuselect options still build app_voicemail and not
res_mwi_external but if both ARE built, res_mwi_external will load
first and become the voicemail provider unless modules.conf rules
prevent it. This is noted in CHANGES.

and support level.
The command "curl -v -u user:pass -X GET 'http://localhost:8088/ari
/asterisk/modules/{moduleName}'" (or something similar, depending on
configuration) can be run in the terminal to access this new
functionality.

ast_module_info->self is often needed to register items with the core. Many
modules have ad-hoc code to make this pointer available to auxiliary sources.
This change updates the module build process to make the needed information
available to all sources in a module.

Add ast_module_shutdown_ref for use by modules that can
only be unloaded during graceful shutdown.
When REF_DEBUG is enabled:
* Add an empty ao2 object to struct ast_module.
* Allocate ao2 object when the module is loaded.
* Perform an ao2_ref in each place where mod->usecount is manipulated.
* ao2_cleanup on module unload.

No functionality change. Just move the definition of ast_module_reload
from _private.h to module.h so it can be public.
Also removed the include of _private.h from manager.c since ast_module_load
was the only reason for including it.

When implementing playback for stasis-http, the monolithicedness of
res_stasis really started to get in my way.
This patch breaks the major components of res_stasis.c into individual
files.
* res/stasis/app.c - Stasis application tracking
* res/stasis/control.c - Channel control objects
* res/stasis/command.c - Channel command object
This refactoring also allows res_stasis applications to be loaded as
independent modules, such as the new res_stasis_answer module.
The bulk of this patch is simply moving code from one file to another,
adjusting names and adding accessors as necessary.

When a module's configuration is not loadable, we still load the module but it
is not in a running state. When trying to troubleshoot, let's say, why
chan_motif is ignoring inbound XMPP traffic, there is no way to indicate that a
loaded module is not currently running.

Prior to this patch, res_musiconhold existed at the same module priority level
as the timing sources that it depends on. This would cause a problem when
music on hold was reloaded, as the timing source could be changed after
res_musiconhold was processed. This patch adds a new module priority level,
AST_MODPRI_TIMING, that the various timing modules are now loaded at. This
now occurs before loading other resource modules, such that the timing source
is guaranteed to be set prior to resolving the timing source dependencies.

Load the proper XML documentation when multiple modules document the same application.
This patch adds an optional "module" attribute to the XML documentation spec
that allows the documentation processor to match apps with identical names from
different modules to their documentation. This patch also fixes a number of
bugs with the documentation processor and should make it a little more
efficient. Support for multiple languages has also been properly implemented.

Modules placed in the priority heap for loading were not properly removed from the linked list. This resulted in some modules attempting to load twice.

This patch adds the option to give a module a load priority. The value represents the order in which a module's load() function is initialized. The lower the value, the higher the priority. The value is only checked if the AST_MODFLAG_LOAD_ORDER flag is set. If the AST_MODFLAG_LOAD_ORDER flag is not set, the value will never be read and the module will be given the lowest possible priority
on load. Since some modules are reliant on a timing interface, the timing modules have been given a high load priorty.

This patch adds 'const' tags to a number of Asterisk APIs where they are appropriate (where the API already demanded that the function argument not be modified, but the compiler was not informed of that fact). The list includes:
- CLI command handlers
- CLI command handler arguments
- AGI command handlers
- AGI command handler arguments
- Dialplan application handler arguments
- Speech engine API function arguments
In addition, various file-scope and function-scope constant arrays got 'const' and/or 'static' qualifiers where they were missing.

This commit introduces the first phase of an effort to manage documentation of the
interfaces in Asterisk in an XML format. Currently, a new format is available for
applications and dialplan functions. A good number of conversions to the new format
are also included.

In this commit:
- move the ast_register/unregister_app functions to module.h
to avoid the need to include pbx.h for the simpler apps;
- move the ast_group structure to channel.h to remove the
dependency of app.h on linkedlists.h
Note, this is a long process that I am doing in small steps.
The main difficulty is that now for each subsystem we
have a single header (e.g. channel.h) included by the subsystem
provider (usually one file, e.g. channel.c) and by its clients
(dozens of them, e.g. we have some 70+ apps and 30+ functions).
This requires the clients to include all the extra headers
required by the provider (eg. lock.h, linkedlists.h, definitions
of substructures...) even though many of the clients would be
just happy with opaque struct declarations and function prototypes.
The long term plan is to eventually rectify this structure
so that the compilation can become faster, and also APIs
are more stable.

To help combat problems where people build external modules (asterisk-addons or others) and then change the build options of the Asterisk build in a way that makes the incompatible without warning, this commit introduces an MD5 signature of the important build-time options and includes that signature into modules when they are built. When the loader loads one of these modules and notices the problem, it will emit a warning to console and refuse to initialize the module, as doing so could cause the system to be unstable or even crash.
If you upgrade to this version of Asterisk, you must rebuild *all* of your modules that came from other sources before trying to run this version. If you are using Digium's G.729 binary codec module, you will need v33 or newer.

universal mod to make ast_flags a 64 bit thing. Instead, I added a 64-bit version of ast_flags (ast_flags64), and 64-bit versions of the test-flag, set-flag, etc. macros, and an app_parse_options64 routine, and I use these in app_dial alone, to eliminate the 30-option limit it had grown to meet. There is room now for 32 more options and flags. I was heavily tempted to implement some of the other ideas that were presented, but this solution does not intro any new versions of dial, doesn't have a different API, has a minimal/zero impact on code outside of dial, and doesn't seriously (I hope) affect the code structure of dial. It's the best I can think of right now. My goal was NOT to rewrite dial. I leave that to a future, coordinated effort.

When shutting down "gracefully", go through and run the unload() callbacks for
all of the modules. "stop now" is considered a non-graceful shutdown and will
not go through this process.

As partly documented in loader.c and include/asterisk/module.h,
modules are now expected to return all of their methods and flags
into a structure 'mod_data', and are normally loaded with RTLD_NOW
| RTLD_LOCAL, so symbols are resolved immediately and conflicts
should be less likely. Only in a small number of cases (res_*,
typically) modules are loaded RTLD_GLOBAL, so they can export
symbols.
The core of the change is only the two files loader.c and
include/asterisk/module.h, all the rest is simply adaptation of the
existing modules to the new API, a rather mechanical (but believe
me, time and finger-consuming!) process whose detail you can figure
out by svn diff'ing any single module.
Expect some minor compilation issue after this change, please
report it on mantis http://bugs.digium.com/view.php?id=6968
so we collect all the feedback in one place.
I am just sorry that this change missed SVN version number 20000!

Everything will still work as before, but new-style modules
can now be defined by putting a '#define STATIC_MODULE' somewhere
before including module.h, then declaring STATIC_MODULE the
various methods (load, unload, key...) that the module is
supposed to supply, and adding a 'STD_MOD(MOD_1, reload_fn, NULL, NULL)'
macro call at the end.
A module compiled in this way will be loaded RTLD_NOW|RTLD_LOCAL
so symbol pollution is reduced, and symbols are resolved immediately.
Removing just the '#define STATIC_MODULE' will restore the old
behaviour.
In order for a module to be loaded RTLD_NOW|RTLD_LOCAL, it must not
export any symbol[1], and all the modules it depends on (e.g. res_*)
must be loaded already.
[1] Mechanisms are in place, and will be enabled later, to still
allow such modules to 'export' symbols and resolving the dependencies
irrespective of the load order.
